The development of antidepressant medications has proceeded through several historical phases. The older antidepressants such as monoamine oxidase inhibitors and early tricyclic antidepressants were discovered largely by serendipity. While effective antidepressants, these medications affected a wide range of neurotransmitter systems and caused many undesirable side effects... Amitriptyline 25mg is demethylated in the liver to nortriptyline, which has antidepressant effects. It is approximately 95 percent protein bound, and has a t 1/2 of 9 to 25 hours. Like imipramine, it potently blocks reuptake of norepinephrine and serotonin. It has higher affinities for histaminic and muscarinic receptors than imipramine; compared with other cyclic antidepressants, amitriptyline is highly sedating and anticholinergic.
Metronidazole is one of the mainstay drugs for the treatment of anaerobic infections and the treatment of choice for most patients with diarrhea caused by Clostridium difficile. It is approved by the United States Food and Drug Administration (FDA) for the treatment of anaerobic and protozoal infections. Metronidazole exerts its antimicrobial effects through the production of free radicals, which are toxic to the microbe. Metronidazole is cytotoxic to facultative anaerobic bacteria, such as Helicobacter pylori and Gardnerella vaginalis, but the mechanism of this action is not well understood...
Mirtazapine—*Data on pregnancy outcomes after exposure to mirtazapine are limited. A prescription events monitoring study reported on 41 cases of first trimester exposure. There were four premature births (26 to 36 weeks of gestation), eight therapeutic terminations, and eight spontaneous miscarriages; one outcome was not known; presumably the other 20 babies were healthy and delivered at term. The study limitations included a lack of control groups, and incomplete information on diagnoses, doses, and duration of treatment. Mirtazapine has also been used in the first and second trimesters for the treatment of hyperemesis gravidarum as described in two small case series and a case report. All of the babies were reportedly normal at delivery except for one case of mild pulmonary hypertension.
